The NHS Chronic Medication Service

We have been working with the Scottish Government Health Directorates on a leaflet outlining a new primary care service for people with a long term condition. The Chronic Medication Service (CMS) will be rolled out across pharmacies in Scotland this month and is a partnership between patients, pharmacists and GPs. The service will help patients with a long term condition manage their medicines better.

Having conducted user testing with patients we are now finalising the information and hope to publish the A5 leaflet in June. An A3 poster advertising the new service will also be displayed in local pharmacies.
